Which Nepal trek is right for you? The short version
Four treks dominate the "which one should I book" question. They are genuinely different trips, not tiers of the same trip, and the right answer depends on what you want rather than on which is hardest.

The headline difference is shape. The Annapurna and Manaslu Circuits are loops that cross a high pass and come down somewhere else. Annapurna Base Camp and Everest Base Camp walk up a valley and come back the same way. That changes the variety of what you see far more than the altitude figures suggest.
Manaslu Circuit vs Annapurna Circuit
The closest comparison, and the one most people on this page came for. Both are true circuits crossing a high pass.

Manaslu is what the Annapurna Circuit was two decades ago: quieter, rougher, less comfortable, and for some people better because of it. The trade-off is real: fewer teahouse options, less reliable food and charging, and a genuine sense of being a long way from help.
The 310 m of extra altitude on Thorong La does not make the Annapurna Circuit the harder trip. Larkya La is the longer day, the descent to Bimthang is bigger and rougher, and a bad day on Manaslu cannot be solved by getting in a jeep. See difficulty and fitness for what that asks of you.
Choose Manaslu if you have trekked in Nepal before and want solitude, a restricted area and fewer boots on the trail. Choose Annapurna if you want comfort, variety and road access at both ends, in which case book our Annapurna Circuit trek instead of this one.
Manaslu Circuit vs Everest Base Camp
The other trek people most often weigh against this one when booking their one big Himalayan trip.

The altitudes are close enough that this is not really a difficulty decision. It is a decision about what you want to have seen, and about shape. Manaslu is a loop that crosses a pass and comes down somewhere else. Everest Base Camp walks up a valley and comes back down the same way.
Everest gives you the mountain everyone has heard of, at the price of a flight that can strand you at either end and a trail that carries far more people. Manaslu gives you a restricted area that most trekkers never see, at the price of simpler lodges and a serious commitment once you are past Jagat.

Manaslu Circuit vs Annapurna Base Camp
Not a close comparison, and that is the useful part. These two sit at opposite ends of what a Nepal trek can ask of you.

Choose Manaslu if you want a pass crossing, real landscape change and a restricted area, and you have two-plus weeks. Choose Base Camp if you have 10 days, want to stand surrounded by 7,000 m walls, or are less certain about high altitude: 4,130 m is a materially smaller ask than 5,106 m, and there is no pass day at all.
The Sanctuary is a genuinely different experience: you finish standing in a bowl with mountains on every side, rather than crossing a pass and descending the other side. If that is the picture in your head, book our Annapurna Base Camp trek instead of this one.

Can you combine treks?
Yes, with enough time. Common combinations:
- Manaslu Circuit + Manaslu Base Camp, the easiest add-on, two extra days in the middle rather than at the end. See our 16-day Manaslu Base Camp itinerary.
- Manaslu Circuit + Tsum Valley, not a second trek but the fullest version of this one, at 19 days.
- Manaslu Circuit + Annapurna Circuit, around four weeks, and the two treks join at Dharapani, so the second one starts where the first finishes.
- Two trips, two seasons, honestly the better plan for most people than one exhausting month.
